I talked previously about self-medication and how in Brazil at least there is a strong culture of self-medication, since not many drugs require a doctor's prescription. Mostly Antibiotics and strong medicines that affect neurotransmitters are the ones that you need prescriptions. For example, I take anticoagulants and in Canada, I need a prescription for that, but in Brazil no. People continue to self-medicate and make their own decisions when feeling a symptom, which can lead to awkward moments, sometimes very risky to our lives. I saw some examples happening in my circle of friends and family.
There is an over-the-counter medicine called "Engov" in Brazil which is the "amulet" of those who like to drink alcohol. I used it when I was younger since there is this tradition and old tv ads selling the medicine saying that it prevents you from having a hangover. After I stopped and studied the composition more, I saw how powerful was the ads to sell a lie. The product mainly have 4 components: Mepyramine maleate, a known anti-histaminic that can cause drowsiness; Aluminium hydroxide, an anti-acid for the stomach; Caffeine, a vaso-constrictor; and Acetylsalicylic acid. Hangover always brings headaches and elevated acid production. That's probably why they added Acetylsalicylic acid (the same as Aspirin) and caffeine to target the headache and the aluminum hydroxide. But Acetylsalicylic acid has a problem, it is a bomb in the stomach. So even with the anti-acid, you are still hurting the stomach with that. The most powerful medicine against to hangover is simple: water. The more water that you take is better, that will reduce your hangover the next day. But how did Engov enter my history?
I was getting married and my wife was having some nausea, probably because of all her stress through the day with her mother and anxiety. The photographer decided to give her Engov, I didn't notice. So after some minutes, she started to vomit, probably because the medicine hurt more her stomach. She has chronic gastritis and for sure the stress of the day affected her a lot and the medicine just helped to get worse.
Another bad example was with my mother-in-law. She is the queen of self-medication. She has some severe obesity problems, such as high blood pressure. I hear once in a while she self-medicating diuretics by herself. But one weird day she was feeling some pain in the neck and headache, and other high blood pressure symptoms. She decided to take a non-steroid anti-inflammatory like naproxen, she got worse of course and we had to take her to the hospital. She was having a very bad peak of high blood pressure. She was luck that they stabilized her pressure there. But it was a very unfortunate decision for her to do that.
The self-medication culture is far to end. You see that happening a lot, people don't like to feel sick and they thing that the answer for that is always behind medicine. Many times people with diarrhea for example, with a virus infection think that reducing the bowel movement is going to fix it like Loperamide (Imodium). But in reality, we need to let it go and just keep hydrated. Depending on what is your decision you can get worse and go to the hospital, maybe your life can be in danger.
Eu falei anteriormente sobre automedicação e como no Brasil pelo menos existe uma cultura forte de automedicação, já que não são muitos os medicamentos que exigem prescrição médica. Principalmente antibióticos e medicamentos fortes que afetam os neurotransmissores são os que você precisa de prescrição. Por exemplo, eu tomo anticoagulantes e no Canadá, preciso de uma prescrição para isso, mas no Brasil não. As pessoas continuam se automedicando e tomando suas próprias decisões quando sentem um sintoma, o que pode levar a momentos estranhos, às vezes muito arriscados para nossas vidas. Vi alguns exemplos acontecendo no meu círculo de amigos e familiares.
Existe um medicamento de venda livre chamado "Engov" no Brasil que é o "amuleto" daqueles que gostam de beber álcool. Depois que parei e estudei mais a composição, vi o quão poderosos eram os anúncios para vender uma mentira. A ressaca sempre traz dores de cabeça e produção elevada de ácido. É provavelmente por isso que eles adicionaram ácido acetilsalicílico (o mesmo da aspirina) e cafeína para atingir a dor de cabeça e o hidróxido de alumínio. Mas o ácido acetilsalicílico tem um problema, ele é uma bomba no estômago. Então, mesmo com o antiácido, você ainda está machucando o estômago com isso. O remédio mais poderoso contra a ressaca é simples: água. Quanto mais água você tomar, melhor, isso reduzirá sua ressaca no dia seguinte. Mas como o Engov entrou na minha história?
Eu estava me casando e minha esposa estava com náuseas, provavelmente por causa de todo o estresse do dia com a mãe e ansiedade. O fotógrafo decidiu dar Engov a ela, eu não percebi. Então, depois de alguns minutos, ela começou a vomitar, provavelmente porque o remédio doía mais no estômago. Ela tem gastrite crônica e com certeza o estresse do dia a afetou muito e o remédio só ajudou a piorar.
Outro exemplo ruim foi com minha sogra. Ela é a rainha da automedicação. Ela tem alguns problemas graves de obesidade, como pressão alta. Ouvi dizer que de vez em quando ela se automedica com diuréticos. Mas um dia estranho ela estava sentindo dor no pescoço e dor de cabeça, e outros sintomas de pressão alta. Ela decidiu tomar um anti-inflamatório não esteroide como naproxeno, ela piorou, é claro, e tivemos que levá-la ao hospital. Ela estava tendo um pico muito ruim de pressão alta. Ela teve sorte que eles estabilizaram sua pressão lá. Mas foi uma decisão muito infeliz para ela fazer isso.
A cultura da automedicação está longe de acabar. Você vê isso acontecendo muito, as pessoas não gostam de se sentir doentes e acham que a resposta para isso está sempre atrás de remédios. Muitas vezes, pessoas com diarreia, por exemplo, com uma infecção viral, acham que reduzir o movimento intestinal vai consertar como Loperamida (Imodium). Mas, na realidade, precisamos deixar isso de lado e apenas nos manter hidratados. Dependendo de qual for sua decisão, você pode piorar e ir para o hospital, talvez sua vida possa estar em perigo.
