On Thursday morning, a middle-aged patient visited me. It was his second visit in the last two weeks, but he had visited our clinic many times before. In the previous two visits, he mostly came with a headache and neck pain. On the last visit, in addition, he had discomforting pain in his upper back as well.
He is an hypertensive patient, on medication. He is hypothyroid and on medication. He is a one-kidney man, the other one was removed due to some pathology. He has a disc problem in his neck. Due to the disc problem, he gets the pain in neck and upper back area quite frequently.
As he is short on one kidney, even though the kidney function is okay, he is quite conscious about taking any kind of strong painkillers like NSAIDs. He only takes paracetamol for any kind of pain in his body. He doesn't take any diclofenac or ibuprofen, even though he feels severe pain!
Every time he gets more pain, not subsided by ointment and oral paracetamol, he visits a doctor to take injectable paracetamol. We have a 1g IV paracetamol injection in Oman and the most used injectable painkiller here. On each visit, he just takes the injection and then leaves! Sometimes, he takes the paracetamol tablet refill too along with a painkiller ointment!
