This carousel which is represented here on the head of the character and not inside (that would have been complicated) brings at least some color in this dark representation of the madness.
Unfortunately this light or what we can imagine as a delirium is often too invasive in the mentally ill and produces symptoms incompatible with a normal social life and leads as here to a forced isolation.
