On an evening shift duty, an above 40 patient was brought by her husband. She was suffering from headaches, neck pain, dizziness, giddiness, etc. I checked her blood pressure and it was the highest blood pressure I ever recorded! Her BP was 270/160 mmHg! I checked multiple times. Every time I found the same blood pressure. I asked them to go to the higher center immediately because I didn’t have an injectable blood pressure-lowering medication except for the Lasix injection. They didn’t want to go to the higher center at first! However, I was able to persuade them. I gave her a tablet of amlodipine 10 mg stat and wrote a short note about the recorded blood pressure and the name of the given medicine to her. They left.
Two days later, the husband visited me, alone. He only came to check his blood pressure. I found his blood pressure was 180/110 mmHg. I asked him about her wife. That night, she was taken to a nearby private hospital. She stayed there for a few hours and did many tests. According to him, no obvious problems were revealed from the tests. She was given a few medications and asked for follow-up.
I told the husband (during his visit) to do some investigations. He denied. I asked him to start a blood pressure-lowering medication. He denied that too. He told me that he would try non-drug management first. I asked him to come for the follow-up. He promised to come for a follow-up and bring his wife also.
