An above-40 patient came with the complaint of severe left lower abdominal pain for the last 2-3 hours. He had no other symptoms; no fever, nausea/vomiting, diarrhea, urinary pain, h/o similar pain! On physical examination, I found the maximum tenderness in the left lower abdominal area just left and below the umbilicus. His features reminded me of another patient who visited me 3 or 4 days before. Both had exactly the same presentation!
I asked the second (latest) patient to do a urine test. I gave injectable painkillers. The urine result revealed numerous RBCs in the it. The finding goes in favor of renal stone. According to the location of his place, the stone was likely to be stuck at the left-sided vesico-ureteric junction. I asked him to do a scan from another hospital. I prescribed him some oral medicine. Then he left.
Around one hour later, the first patient (who came to my mind while consulting the latest/second patient of this story) came to show the scan report on the same day. Because I also asked him to do an ultrasound scan to confirm the diagnosis! It was a case of a left-sided renal stone with an impacted stone in the left vesico-ureteric junction! My initial diagnosis of him (first one) was correct!
The second patient returned with a scan report the next day. It was a case of both-sided renal stones with an impacted stone in the left vesico-ureteric junction! My initial diagnosis of him (second one) was correct too!
